CLIENT CARE AND SERVICE:

HOV is committed to helping clients remain in their own homes and to preserving independence in their lives. In addition, HOV provides a link to additional community resources and services through referral. Whether we provide transportation, in-home services or referral services, HOV maintains high values of integrity, confidentiality and credibility with clients. All services will be performed in a prompt and efficient manner and if follow up is required it shall be done on a timely basis. Services will be provided in such a way as to encourage participants to sustain their optimal level of independent living.

WHAT WE DO:

1) Coordinate services between clients and volunteers. This may include:

  • contact with client, family and other care and service providers
  • utilization of any of the following services:
  • Escorted Transportation
  • Visitation/Companionship (both in person and over the telephone)
  • Respite care
  • Light Yard work
  • Handyman Services
  • Assistance with health insurance forms
  • Letter writing/paperwork
  • Telephone calling
  • Assistance with pets
  • Shopping and Errands (Take client or perform shopping and errands for client)
  • Advocacy

2) Provide Information and referral services:

  • Provide a link between available supportive social services and older adults, their families and friends.
  • Maintenance of reference material, community contacts, and established and approved care provider organizations.
  • Network with other organizations and agencies.
  • Refer client to appropriate service provider.
  • Supply appropriate and accurate authorized information as needed by client.

WHAT WE DO NOT PROVIDE:

  • Regular weekly or ongoing services
  • Services before 9am or after 9pm
  • Transportation for wheel-chair clients
  • Transportation to surgical or procedural appointments
  • Moving services
  • Taxi service
  • Regular housecleaning services
  • On-going gardening services
  • On-going meal preparation
  • Nursing care or medical/health services
  • Personal Care Services
  • Mental health services
  • Legal services, representation, or assistance
  • Financial assistance or loans
  • Medical equipment
