CLIENT INFORMATION REQUIREMENTS

Client must be 59 or older and live in the HOV service area.

HOV will have a current and complete intake sheet on each client. Name, address, and phone number should be obtained from each caller.

Client will be asked for a date, pickup time, and place of service.
