Heart of the Valley Joins the Able People Foundation to give a Gift of Mobility this Holiday Season
Heart of the Valley and the Able People Foundation have teamed up to provide the gift of mobility this holiday season. We will be providing limited quantities of bathtub transfer benches for free. The bathtub transfer benches are designed to be placed half inside and half outside the bathtub for increased security while transferring in and out of the bathtub or shower.
To qualify, you must:
- Complete and send attached application before December 1, 2010.
- Send necessary documents as stated on the application.
- Must be living at home and not at a care facility.
- Receive less than $1,700 monthly. "Proof of income required."
- Live in the Bay Area, thereby giving you access to our giving location for setup and pickup.
- Your insurance will not pay for a bathtub transfer bench.
How it works:
- You must apply for a free bathtub transfer bench by sending us your application and required documents.
- A committee will review your application.
- We will mail you a letter about our review and decision.
- If approved, our letter will include date, day, and location within Santa Clara County to go and receive your Gift of Mobility - Free Bathtub Transfer Bench.
